Birth and Death Records
The Vital Statistics Department of the City of Middletown Health Department can issue Birth certificates and Death certificates for anyone who was born or died anywhere in the State of Ohio. Birth and Death certificates cost $25.00 per copy. We accept CASH and CREDIT with the exception of American Express. We do not have tap pay and there is no ATM in the city building.
Payment is required at time of order.
Certificates will be sent via USPS at no charge. Certificates can be expedited with a tracking number for an additional charge. You can print an order form below and fill it out before you arrive or you can fill an order form out at our office.
Birth and Death certificates are printed on State issued security certificate paper. They cannot be copied or laminated.
If you were born in a different state, you must contact a health department in the state you were born to obtain a copy of your birth certificate.
Marriage Licenses
Marriage licenses are NOT available at the City of Middletown Health Department. In Ohio, certified copies of your marriage license can only be obtained from the probate court in the county where your marriage license was originally issued. Please call ahead to check hours of operation for your corresponding county.

Home Births
Parents who birth their baby at home or outside a birthing facility, will register the birth of their baby with the local vital statistics office within ten (10) days of the baby’s birth. You will need to request a home birth packet from the Vital Statistic Office by calling (513) 425-7851. This can be done by the licensed midwife or the parents. It is highly recommended that you obtain this packet in advance of the birth to avoid being rushed to get all of the required information in a short amount of time. Once the certificate has been prepared after the birth packet is filed and reviewed by the Ohio Department of Health, the parents may purchase a certified copy of the birth certificate.
You must register a home birth with the local health department where the home birth occurred. If your home birth occurred in Middletown, Ohio is advisable that you contact our office prior to your visit and schedule a time to review the documentation with the registrar. Adoption Records
Adult adoptees may submit an application to the Ohio Department of Health for a copy of their adoption file. These records cannot be purchased at the City of Middletown Health Department.
