Services are provided in accordance with State of Ohio statutes and Ohio Public Health Council directives.
Vital Statistics
The Vital Statistics Department of the City of Middletown Health Department can issue Birth certificates and Death certificates for anyone who was born or died anywhere in the State of Ohio. Birth and Death certificates cost $25.00 per copy. We accept CASH and CREDIT with the exception of American Express. If you were born in a different state, you will need to contact a local or county health department in the state you were born to receive your birth certificate. Environmental Health
The Environmental Department assures a Safe and Healthy environment by monitoring and licensing all food related facilities: restaurants, grocery stores, vending locations, mobile units and temporary events, like the Ohio Balloon Challenge. The Environmental Department investigates food borne illnesses within the city limits of Middletown and has responsibilities regarding private water supplies, new installation inspections of on-site sewage systems and surveys. The Environmental Department monitors school environments, tattoo and body piercing establishments, provides smoking ban enforcement as mandated by Ohio State Law and inspects the City Jail. The Environmental Department investigates animal bites for the Rabies Control Program and licenses and monitors all pools and spas (apartment complexes, health clubs etc.). The Environmental Department is also very involved in Emergency Response Planning and Emergency Response Training activities in Ohio. Health Education the City of Middletown Health Department provides information to the community regarding a wide variety of health issues, environmental food safety, health wellness programs, preparedness and current public health events and alerts impacting our Community, County and State.
